DexaFit Terms and Conditions

1. Use and Disclosure of Protected Health Information
I consent to allow Burning River Physical Therapy and Wellness DBA DexaFit Cleveland, to use their DXA scanner to perform a body composition and/or bone densitometry scan.
Radiation Notice: I am fully aware that the technology uses low-dose X-rays.
Bone Densitometry: This scan is "Tech Only," is not read by a radiologist at DexaFit, and will not be reviewed with the client.
2. Records Review for Research
I authorize Burning River Physical Therapy and Wellness DBA to review my records to determine if my body scan qualifies for approved clinical studies and to contact me if I am a potential candidate.
Privacy Guard: No records will be shared with third parties for research purposes without my specific written approval.
3. Financial Responsibility
I accept full financial responsibility for all charges for services provided to me and/or my family members.
Authorization: I verify that I am the cardholder and authorize DexaFit to charge the card on file for purchases, no-show fees, and late cancellation/rescheduling fees.
Collections: If the card on file cannot be charged, fees must be paid immediately and may be subject to collections.
Refunds & Expirations: * No refunds on tests rendered.
All packages expire 12 months from purchase.
No partial refunds on unused tests.
Promotions: Vouchers or discount codes must be applied at the time of scheduling; no retroactive refunds will be issued.
4. Facility and Cancellation Policies
Facility Policy: Guests, children, or pets are not allowed. If they arrive with you, they will be asked to leave, and your appointment may be canceled with a late fee applied.
Cancellation/No-Show: Notifications must be made 24 hours in advance.
Late Fees: A $75 fee applies per test scheduled for late cancellations, no-shows, or arrivals more than 10 minutes late.
5. Mobile Messaging and SMS Terms
By providing your mobile number and opting in, you agree to the following:
Purpose: Automated transactional and/or marketing text messages regarding appointments, results, and wellness offers.
Consent: Consent is not a condition of purchase.
Fees: Message and data rates may apply.
Opt-Out: Unsubscribe at any time by replying STOP, END, CANCEL, UNSUBSCRIBE, or QUIT.
Support: Reply HELP for assistance or contact [email protected].
“As always, message and data rates may apply for any messages sent to you from us and to us from you. Message frequency may vary. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.”
6. Waiver and Agreement
I hereby release Burning River Physical Therapy and Wellness DBA DexaFit Cleveland from any liability for injury or damages. I assume all risks of injury or death resulting from DXA scans, RMR Analysis, or VO2 Max testing.
Health Declaration: I certify I am physically sound, not pregnant, and have obtained (or waived) physician approval.
No Diagnosis: I understand DexaFit does not diagnose or interpret scan results; these should be reviewed with a primary care physician.
7. Age Restriction (18+)
No Minors: All clients must be at least 18 years old. We cannot perform scans on individuals under 18, regardless of parental consent.
ID Required: Please bring a valid photo ID to your first appointment to verify eligibility.
8. Client HIPAA Consent & Authorization
I understand my rights under HIPAA and authorize the use of my health information for treatment, payment, and operational purposes.
Release of Info: I authorize the release of my health and fitness information to myself or authorized parties via email, fax, mail, or the DexaFit private login.
Revocation: This authorization is valid until revoked in writing to [email protected].
9. Carrier Liability Clause
"Carriers are not liable for delayed or undelivered messages."
